Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist to deliver patient-centered care in home settings. You will assess functional status, develop treatment plans, and collaborate with families and a multidisciplinary team to restore independence and safety for daily living activities.

Responsibilities include fabricating splints, supervising COTAs, educating patients and caregivers, and coordinating discharge planning to ensure smooth transitions.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT registration.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience preferred (home health experience a plus).
• Current CPR certification.

Responsibilities

• Evaluate patients’ functional status and home safety.
• Assess home environments and modify or adapt equipment.
• Develop individualized, measurable treatment plans.
• Provide therapeutic interventions to restore function.
• Fabricate splints and train on adaptive devices.
• Support return-to-work through functional/work capacity evaluations.
• Educate patients and caregivers on plans and safety.
• Coordinate with care team for seamless patient-centered care.
• Participate in discharge planning to support transitions.
