Job summary

Ohio Living is seeking a dedicated Occupational Therapist to provide skilled OT services in patient homes and care facilities across Ohio. You will evaluate patients, develop individualized care plans, and coordinate with physicians and the care team to maximize function and safety.

Qualifications

• Current unencumbered Ohio OT license.
• 1 year of OT experience preferred; new graduates welcome.
• Experience in geriatric care and OT patient care.
• Knowledge of OT treatment modalities.

Responsibilities

• Perform OT evaluations and treatments per physician orders.
• Develop and implement the Plan of Care and discharge planning.
• Educate patients and families on exercises and safety.
• Collaborate with interdisciplinary team to ensure continuity of care.
• Maintain accurate patient records in EMR and charts.
• Assist in education and onboarding of staff as needed.
