Ohio Living is seeking an Occupational Therapist to perform evaluations and treatments for patients in home health settings across our Ohio communities. You will ensure care meets physician orders and complies with federal, state, and local standards while delivering high-quality, compassionate therapy.

The role focuses on function, safety, and discharge planning, with emphasis on family education, teamwork with the interdisciplinary group, and maintaining accurate records using EMR and charts.

Qualifications

• Current unencumbered Ohio OT license.
• One year hospital/long-term care/home health experience preferred. New grads welcome.
• Experience in geriatric care and OT evaluation/treatment modalities.

Responsabilités

• Care Management and Service Delivery - 60%: Initiates orders, conducts evaluations, and develops treatment plans.
• Performs home health OT treatments and assesses patient safety during care.
• Adjusts Plan of Care based on progress and physician guidance.
