Job summary

VitalCaring Group is seeking a dedicated Occupational Therapist to join our Crystal River Home Health team. This field-based role focuses on helping patients regain independence and optimize daily functioning in the home setting.

You will collaborate with families and interdisciplinary team members to deliver individualized, high-impact care, including ADL retraining, safety assessments, and adaptive equipment recommendations.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT registration.
• Current state licensure as an Occupational Therapist.
• Minimum of one year clinical experience; home health a plus.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status, including mobility, strength, coordination, communication, and ability to perform daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life.
• Provide therapeutic interventions to restore function, including retraining in daily living activities.
• Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate.
• Support patients returning to work through functional and work capacity evaluations.
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure seamless, patient-centered care.
• Participate in discharge planning to support successful transitions and continued progress.
