Job summary

Qualifications

• Graduate of an accredited Occupational Therapy program (OTR accepted).
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist.

Responsibilities

• Assess patients' functional status and home safety.
• Develop individualized home-based treatment plans with measurable goals.
• Provide therapeutic interventions to restore daily living activities.
• Fabricate splints and train use of adaptive devices.
• Support return-to-work through functional/work capacity evals.
• Educate patients and families on plans, safety, and care needs.
• Collaborate with care team to coordinate patient-centered services.
• Participate in discharge planning for smooth transitions.
