Zusammenfassung

VitalCaring Group is seeking a dedicated Occupational Therapist to join our home health team serving the Crystal River area. This field-based role focuses on helping patients regain independence and safely manage daily activities in their homes.

You will collaborate with patients, families, and interdisciplinary teams to deliver personalized, high-impact care, develop treatment plans, and educate caregivers while supporting transitions and continued progress.

Qualifikationen

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT registration.
• Current state licensure as an Occupational Therapist.
• Minimum of one year clinical experience preferred (home health experience a plus).
• Current CPR certification.
• Valid driver’s license, reliable transportation, and auto insurance.

Aufgaben

• Evaluate patients’ functional status and daily living activities.
• Assess home environments for safety and adaptive equipment needs.
• Develop individualized treatment plans with clear, measurable goals.
• Provide therapeutic interventions to restore function and retrain in daily living activities.
• Fabricate splints and train patients in the use of adaptive devices.
• Support patients returning to work through functional/work capacity evaluations.
• Educate patients, families, and caregivers on plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure patient-centered care.
• Participate in discharge planning to support transitions and ongoing progress.
• Supervise COTAs and oversee home health aide care when required.
