Job summary

VitalCaring Group is seeking a licensed Occupational Therapist to join our home health team in Florida. In this field-based role, you will help patients regain independence, assess living environments, and create individualized treatment plans to improve daily function at home.

You'll collaborate with families and the care team, supervise COTAs, and participate in discharge planning to ensure smooth transitions. NBCOT and state licensure required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification is required.
• Current state OT licensure is required.
• Minimum 1 year home health experience preferred.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients' functional status and daily living activities.
• Assess home environments for safety and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with measurable goals.
• Provide therapeutic interventions to restore function and retrain daily living tasks.
• Educate patients, families, and caregivers on plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure seamless patient-centered care.
• Supervise COTAs and oversee home health aide care as required.
• Assist with discharge planning to support transitions.
