Job summary

VitalCaring Group in the United States is seeking a dedicated Occupational Therapist (OT) for Home Health. This full-time, field-based role focuses on helping patients regain independence and safely perform daily activities at home.

You will assess functional status, design individualized treatment plans, fabricate splints when needed, and collaborate with families and the care team to ensure seamless, patient-centered care.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience; home health experience a plus.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Evaluate patients’ functional status and home mobility.
• Assess home environments for safety and needed adaptations.
• Develop individualized treatment plans with measurable goals.
• Provide interventions to restore function and retraining in daily activities.
• Fabricate splints and train use of adaptive devices.
• Support return-to-work through functional/work capacity evaluations.
• Educate patients, families, and caregivers on plans and safety.
• Coordinate care with the interdisciplinary team for seamless service.
• Participate in discharge planning to support progress.
