Ohio Living in Ohio is seeking an Occupational Therapist to perform evaluations and treatments in alignment with physician orders. The role includes home health and facility-based care to support patients in returning to their highest level of function.

Ideal candidates hold an active Ohio OT license, with 1 year of experience in geriatric care within hospital, long-term care, or home health settings. New graduates are welcome and will be supported by an interdisciplinary team.

Qualifications

• Current Ohio OT license required.
• Experience in hospital, long-term care, or home health preferred.
• Experience with geriatric patients and OT modalities.
• Strong communication with patients, families and staff.
• Ability to read and follow physician orders and treatment plans.

Responsabilités

• Initiates physician contact for orders and treatment plans as patient condition requires.
• Performs OT evaluations and treatments per physician orders and standards.
• Educates patients and families on exercises and safe self-care activities.
• Participates in discharge planning to ensure continuity of care.
• Maintains patient records and documentation per guidelines.
• Collaborates with interdisciplinary team to optimize patient outcomes.
