Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist to provide in‑home rehabilitation, assess daily living skills and design retraining programs to restore strength, mobility and independence for patients in their homes. You will collaborate with physicians and the care team, document care plans and discharge summaries, and participate in coordination of care.

Requires OT license, CPR certification and a valid driver's license.

Qualifications

• Current and unrestricted OT licensure.
• CPR certification required.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs in patients' homes.
• Confer with physician and health care team to develop/revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination and range of motion.
• Review/revise retraining programs to meet patient goals within rehab guidelines.
• Coordinate with vocational team members to support patient needs.
• Document patient status, interventions and discharge summaries for care management.
