Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to provide in-home rehabilitation, helping patients regain strength, mobility and independence within their own homes. You will assess daily living skills, design retraining programs and apply therapy that improves function and safety.

Collaborating with physicians and the care team, you will update plans of care, document progress and support discharge planning.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver''s license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient''s daily living/work-related skills and develop therapeutic retraining programs.
• Confer with the patient''s physician and care team to develop or revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function.
• Document patient status and progress; submit plans and discharge summaries to supervisor.
