Zusammenfassung

CenterWell Home Health is seeking an Occupational Therapist to deliver rehabilitation services in patients' homes, focusing on improving strength, mobility and independence. You will assess daily living skills, collaborate with physicians and the rehab team, and help patients regain independence through individualized therapy plans.

The role requires an active OT license, CPR certification, and a valid driver’s license with reliable transportation.

Qualifikationen

• Current and unrestricted occupational therapy licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Aufgaben

• Assess patient’s daily living/work-related skills and develop therapeutic retraining programs.
• Consult with physicians and rehab team to plan care and revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination and function.
• Document patient progress and coordinate discharge planning with care team.
• Supervise and mentor OT staff and assistants as needed.
