Job summary

CenterWell Home Health is seeking a dedicated Occupational Therapist to support patients at home with personalized rehabilitation. You will assess daily living skills, design retraining plans, and collaborate with physicians to optimize the Plan of Care and patient outcomes.

You will deliver therapeutic treatment per physician orders, advance functional goals, and coordinate with OT Assistants and Home Health Aides to ensure comprehensive care in home settings.

Qualifications

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care.
• Provide therapeutic treatment and instruction to patients in accordance with physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the physician and health care team members to develop or revise the Plan of Care.
• Provide therapeutic treatment and instruction to improve strength, coordination, ROM and function.
• Review/revise retraining programs aligned with patient goals and rehab plan.
• Coordinate/oversee OT Assistant and Home Health Aide performance in implementing OT services.
