Job summary

CenterWell Home Health is seeking a qualified Occupational Therapist to join our caring team. You will assess patients in their homes, develop retraining plans, and work with physicians and the care team to improve function and independence.

You will administer therapies to restore strength, range of motion and coordination, document progress, and participate in discharge planning and care coordination. A valid OT license and CPR are required.

Qualifications

• Current and unrestricted OT licensure in the state.
• Current CPR certification.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Assess and screen patients' daily living and work-related skills and develop therapeutic retraining plans.
• Collaborate with physicians and health-care team to develop the Plan of Care.
• Provide treatments to improve strength, coordination, range of motion and function.
• Document patient status, progress and discharge summaries.
• Design community reintegration activities and coordinate with the interdisciplinary team.
