Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist to deliver in-home rehabilitation, helping patients regain strength, mobility and independence through personalized therapy plans.

You will assess and retrain daily living skills, collaborate with physicians and the care team, document progress and support patients and families in returning to meaningful activities; requires OT license, CPR, and a valid driver's license.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Assess/screen patients' daily living and develop retraining programs.
• Confer with physicians and care team; participate in plan of care development.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function.
• Document care observations and updates to plans of care and discharge summaries.
• Supervise OT Assistant and Home Health Aide performance; educate staff and family.
