Ohio Living is seeking a licensed Occupational Therapist to evaluate and treat patients in our home health and related care settings across Ohio. You will develop and adjust plans of care, coordinate with physicians, and educate patients and families to improve function and safety at home.

Qualifications include an Ohio OT license, 1 year of experience preferred in hospital, long-term care, or home health, with new graduates welcome. Valid driver's license and insurability required.

Formación

• Current unencumbered license for the state of Ohio to practice as an Occupational Therapist (OT) is required.
• One year experience as an Occupational Therapist in a hospital, long-term care facility, or home health agency preferred.
• New graduates welcome.
• Experience in geriatric and occupational therapy patient care is required.

Responsabilidades

• Initiates contact with physicians for orders and care plan formulation or revision.
• Performs home health OT treatments and evaluates patients in home environment.
• Educates patients, families, and other health team members on exercises and self-care to improve safety.
• Maintains current patient records and documentation per guidelines.
• Participates in discharge planning and interdisciplinary team meetings to ensure continuity of care.
