Job summary

VitalCaring is seeking a Home Health Occupational Therapist (OT) to support patients in their homes, collaborating with families and interdisciplinary teams to restore independence and safety in daily activities.

You will evaluate functional status, modify home environments, and develop individualized treatment plans with clear goals to improve quality of life and independence. Leadership duties include supervising COTAs and guiding home health aides when needed.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state OT licensure.
• One year of clinical experience preferred (home health a plus).
• Current CPR certification.

Responsibilities

• Evaluate patients’ functional status and daily activity ability.
• Assess home environments for safety risks and recommend modifications.
• Develop individualized treatment plans with measurable goals.
• Provide therapeutic interventions to restore function and retraining in daily living activities.
• Fabricate splints and train use of adaptive devices.
• Support return-to-work through functional/work capacity evaluations.
• Educate patients, families, and caregivers on plans and safety.
• Collaborate with care team for seamless, patient-centered care.
• Participate in discharge planning.
