Job summary

VitalCaring is seeking a Home Health Occupational Therapist to help patients regain independence in the home setting. You will partner with patients, families, and the care team to deliver personalized, high-impact therapy in field-based environments.

The role emphasizes safety, functional improvement, and discharge planning, with supervision of COTAs and collaboration across disciplines. A valid OT license, NBCOT certification, and CPR are required, along with a compassionate, patient-centered

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification and state OT licensure required.
• Minimum 1 year home health experience preferred.

Responsibilities

• Evaluate patients’ functional status and safety at home.
• Develop individualized treatment plans to improve independence.
• Provide therapeutic interventions and ADL retraining.
• Fabricate splints and train use of devices as appropriate.
• Coordinate with care team and update EMR promptly.
• Educate patients and families on care plans and safety.
