Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist (OT) for field-based, full-time work focused on patient-centered care. You will help patients regain independence and collaborate with families and the care team to deliver personalized plans.

Evaluate functional status, modify home environments, create measurable goals, supervise COTAs, and participate in discharge planning, all while upholding compassionate, safe care and ongoing professional growth.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification.
• Current state OT licensure.
• CPR certification.
• Valid driver's license and reliable transportation.
• 1 year of home health or clinical experience preferred.

Responsibilities

• Evaluate functional status and daily living activities.
• Assess home environments for safety risks and recommend modifications.
• Develop individualized treatment plans with clear, measurable goals.
• Provide therapeutic interventions to restore function.
• Fabricate splints and train use of adaptive devices as appropriate.
• Support return-to-work through functional/work capacity evaluations.
• Educate patients and caregivers on plans, safety and ongoing care needs.
• Document care in EMR and coordinate with care team.
• Supervise COTAs in accordance with state regulations.

What You’ll Do

As a Home Health Occupational Therapist , you will play a critical role in helping patients regain independence, improve daily function, and safely navigate life at home. You’ll partner closely with patients, families, and interdisciplinary teams to deliver personalized, high-impact care.

Patient Care & Clinical Impact

• Evaluate patients’ functional status, including mobility, strength, coordination, communication, and ability to perform daily activities
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment
• Develop individualized treatment plans with clear, measurable goals focused on improving independence and quality of life
• Provide therapeutic interventions to restore function, including retraining in daily living activities
• Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices as appropriate
• Support patients returning to work through functional and work capacity evaluations
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs
• Collaborate with the care team to coordinate services and ensure seamless, patient-centered care
• Participate in discharge planning to support successful transitions and continued progress

Care Coordination & Communication

• Document patient care accurately and timely within the electronic medical record (EMR)
• Communicate plans of care with physicians and interdisciplinary team members
• Participate in case conferences, team meetings, and ongoing clinical collaboration
• Maintain strong, professional relationships with patients, families, and care team members

Leadership & Oversight

• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations and company policy
• When therapy is the sole skilled service, provide oversight of home health aide care as required

Requirements

• Graduate of an accredited Occupational Therapy program
• Current registration with the National Board for Certification in Occupational Therapy (NBCOT)
• Current state licensure as an Occupational Therapist
• Minimum of one (1) year of clinical experience preferred (home health experience a plus)
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
