Job summary

CenterWell Home Health is seeking an Occupational Therapist to provide in-home rehabilitation services, helping patients regain strength and independence in their own homes. You will collaborate with physicians and a care team to implement and adapt therapy plans, coordinate discharges, and support family members in the home care process.

Qualifications

• Current and unrestricted OT licensure.
• CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess and screen patient’s daily living/work-related skills and develop therapeutic retraining programs.
• Collaborate with physician and health care team to develop Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, ROM and function.
• Review and expand retraining programs to meet goals of rehab plan.
• Consult with vocational team to tailor programs to patient needs and capabilities.
• Report patient status and progress to physician, rehab staff and family.
• Participate in care coordination and discharge planning activities.
• Design community reintegration activities and coordinate plans with interdisciplinary team.
• Recommend adaptive equipment to improve home or work environments.
• Design and train staff and family in retraining program activities.
• Supervise and evaluate OT Assistant and Home Health Aide performance.
