Job summary

CenterWell Home Health seeks a compassionate Home Health Occupational Therapist to help patients regain strength and independence in their own homes through personalized rehabilitation and functional training.

You will collaborate with physicians and a multidisciplinary team to assess daily living skills, design retraining programs, document care, and support discharge planning, while guiding patients and families in home-based care practices.

Qualifications

• Current and unrestricted occupational therapy licensure.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with physician and health care team; participate in Plan of Care Treatment.
• Provide therapeutic treatment per physician orders to improve/restore strength, coordination and function.
• Review/expand retraining programs aligned with rehabilitation goals.
• Consult with vocational team to develop plans aligned with patient needs.
• Report patient status to physician and care management staff.
• Participate in care coordination and discharge planning and support team.
• Design community reintegration activities and coordinate with interdisciplinary team.
• Recommend adaptive equipment to improve home/work environments.
• Train staff and family to carry out retraining programs (dressing, feeding, grooming, hygiene).
• Supervise and evaluate OT Assistant and Home Health Aide performance.
