CenterWell Home Health is seeking an Occupational Therapist to work with patients in home settings, focusing on rehabilitation and functional improvement. You will assess, plan and deliver therapeutic interventions to improve independence and quality of life, collaborating with physicians and the care team.

This role emphasizes home-based evaluation, hands-on rehabilitation, and coordination with the interdisciplinary team to ensure goals are met and the patient achieves better daily functioning.

Qualificações

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsabilidades

• Assess/screen patients' daily living and work-related skills and develop retraining programs with measurable objectives.
• Administer and interpret functional assessments to determine patient function levels in home settings.
• Provide therapy to improve/restore strength, coordination, range of motion and functional abilities.
• Document patient status, progress and treatment plans accurately and on time.
• Collaborate with physicians and care teams to update plans of care and discharge planning.

Conhecimentos

OT licensure

CPR certification
