Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist to assess, plan and implement rehabilitation programs in patients' homes. You will collaborate with physicians and the rehab team to design retraining plans that improve independence and daily functioning, while guiding staff and documenting progress.

The role emphasizes home-based evaluation, treatment, wheelchair and adaptive equipment considerations, and coordination with care management to support optimal outcomes.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license with auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop retraining programs with measurable objectives.
• Administer and interpret functional tests at home to assist physician in evaluating function.
• Collaborate with the physician and care team to update the Plan of Care/Treatment.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function.
• Review retraining programs and align with rehabilitation goals and program guidelines.
• Supervise OT Assistant and Home Health Aide in service delivery.
• Accurately document care observations and progress daily.
• Submit evaluations and discharge summaries to supervisors and care management.
