CenterWell Home Health is seeking a Home Health Occupational Therapist to help patients regain independence at home. You will assess, plan and implement rehabilitation programs, coordinate with physicians and the rehab team, and support patients and families throughout care and discharge.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patients' daily living and work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with physicians and rehab team to refine the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, ROM and function.
• Review and expand retraining programs to align with patient goals and rehab guidelines.
• Consult with vocational team members to develop employment-related plans.
• Report patient status and progress to physicians and care team.
• Participate in care coordination and discharge planning.
• Design community reintegration activities and coordinate with the team.
• Recommend adaptive equipment to improve home or work environments.
• Design and train staff and family to carry out retraining activities.
• Supervise OT Assistant and Home Health Aide performance.
• Document care observations, interventions and evaluations accurately.
