Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist to support patients in their homes, helping them regain strength, mobility and independence through rehabilitation and functional improvement.

As part of a multidisciplinary team, you will assess daily living skills, develop retraining programs, coordinate care with physicians, and educate patients and families to promote home-based rehabilitation.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient’s daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient’s physician and health care team and participate in Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, range-of-motion and function.
• Review/expand retraining programs to meet patient goals within the rehabilitation program.
• Design community reintegration activities to support physical reconditioning and psychological adjustment.
