Job summary

CenterWell Home Health is seeking an Occupational Therapist to provide in-home therapy, evaluating daily living skills and creating individualized retraining plans to restore function. You will collaborate with physicians, nurses and other therapists to implement treatment that improves strength and mobility while promoting independence in a home care setting.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with physician and care team to develop or revise the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, ROM and function as ordered by the physician.
• Document care observations, progress, and plans for discharge.
