Job summary

VitalCaring Group in Crystal River, FL is seeking a Home Health Occupational Therapist (OT) for PRN coverage. You will assess patients at home, design individualized treatment plans, and collaborate with families and the care team to restore independence and safety.

This role focuses on patient-centered care and seamless care coordination. Requirements include accredited OT education, NBCOT, state OT licensure, CPR, and 1+ year of clinical experience (home health a plus).

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience preferred (home health a plus).
• Current CPR certification.

Responsibilities

• Evaluate patients’ functional status including mobility, strength, coordination, communication, and daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with clear, measurable goals to improve independence and quality of life.
• Provide therapeutic interventions to restore function, including retraining in activities of daily living.
• Fabricate splints and train patients in use of adaptive devices as appropriate.
• Support patients returning to work through functional/work capacity evaluations.
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure patient-centered care.
• Participate in discharge planning to support transitions and continued progress.
