Job summary

VitalCaring Group seeks a dedicated Occupational Therapist for home health to help patients regain independence and daily function in their homes. You will collaborate with families and interdisciplinary teams to deliver personalized, high-impact care.

Requirements include OT degree, NBCOT certification, state licensure, and CPR. A valid driver’s license and reliable transportation are essential for field-based work across our service area.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification and state OT licensure.
• Minimum of one year clinical experience; home health is a plus.
• Current CPR certification and valid driver’s license.

Responsibilities

• Evaluate patients’ functional status and daily living activities.
• Assess home environments for safety and recommend modifications.
• Develop and implement individualized treatment plans with measurable goals.
• Provide therapeutic interventions to restore function and retraining in daily activities.
• Fabricate splints and train use of adaptive devices as appropriate.
• Support patients returning to work through functional evaluations.
• Educate patients and families on plans, safety, and ongoing care needs.
• Document care in EMR and coordinate with physicians and the care team.
• Participate in discharge planning to support progress.
