Your Health is seeking a Nurse Practitioner to deliver high-quality care to patients in Cheraw and surrounding areas. You will work as part of an interdisciplinary team, evaluating high-risk patients, coordinating care plans, and providing care management in homes, facilities, clinics, and telehealth settings.

The role is full-time with salary-based compensation and a monthly bonus opportunity. Shifts include 12-hour and 8-hour options, with travel between offices as needed.

Qualificações

• General clinical care and patient management experience.
• Experience in home health, telehealth, clinics, or facility-based care.
• Strong teamwork and communication within an interdisciplinary team.

Responsabilidades

• Evaluate high-risk patients with complex medical needs and develop care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity.
• Communicate with case managers, clinicians, social workers, and pharmacists to coordinate patient care.
• Participate in interdisciplinary team meetings to discuss patient progress and plans.
• Document care management activities accurately to support care plans and compliance.
• Provide education and resources to patients about conditions and self-management.
• Adhere to HIPAA and Medicare guidelines and other regulations.
• Travel between offices and facilities as needed to serve patients.
