Job summary

Your Health Organization in South Carolina is seeking a Nurse Practitioner to serve patients in Cheraw and surrounding areas. You will provide home health care, collaborating with an interdisciplinary team to evaluate high-risk patients and coordinate comprehensive care plans.

This full-time role offers a salary-based base with a monthly bonus, requires flexible hours, and travel between offices, facilities, and patient homes to deliver quality primary care.

Qualifications

• NP/PA license in good standing required.
• Experience in home health, clinic, facility, telehealth, or specialty care setting preferred.
• Strong communication, collaboration, and teamwork skills.
• Ability to evaluate high-risk patients, develop care plans, and coordinate care.
• Commitment to patient-centered care and excellence in healthcare delivery.
• Strong organizational and time management skills; multitasking ability.
• Ability to read and communicate effectively; strong written and verbal skills.
• Basic computer knowledge.
• Leadership capability and adaptability in changing environments.
• Flexibility to work hours and travel between offices.
• Must be able to work across various settings (homes, clinics, facilities).

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care activities with interdisciplinary team.
• Communicate with case managers, clinicians, pharmacists, and health coaches.
• Participate in team meetings to discuss patient progress and plans.
• Document care management activities accurately for compliance.
• Educate patients and families about conditions, treatments, and self-management.
• Provide guidance to promote patient participation in care.
• Adhere to HIPAA, Medicare guidelines, and regulations.
• Support quality measures and identify improvement opportunities.
• Travel between offices and facilities as needed.
