Job summary

Your Health in the Columbia area is seeking a Nurse Practitioner to provide in-home primary care. You will join a care team delivering high-quality patient management across homes, facilities, clinics, and telehealth, collaborating with physicians and staff to ensure coordinated care and positive outcomes.

The role is full-time with a monthly bonus potential, serving designated SC service areas and involving shifts in 12-hour or 8-hour formats.

Qualifications

• Licensed Nurse Practitioner in-state with active license.
• Experience in patient care, preferably home health, clinics, telehealth, or facility settings.
• Strong communication and collaboration within an interdisciplinary team.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care activities with the interdisciplinary team to ensure continuity.
• Communicate with case managers, coordinators, specialists, pharmacists, and other team members.
• Participate in team meetings to discuss progress and plans.
• Document care management activities accurately for regulatory compliance.
• Educate patients and families about conditions, treatment plans, and self-management.
