Your Health Organization in South Carolina seeks a Nurse Practitioner to serve patients in Cheraw and surrounding areas. The role covers home health, clinics, and telehealth, with collaboration across care teams to manage high-risk patients and optimize outcomes.

The position is full-time with a monthly bonus opportunity, standard 12-hour or 8-hour shifts, and a strong emphasis on compassionate, patient-centered care and compliance with healthcare regulations.

Qualifications

• Licensed NP in good standing in SC; home health/clinic experience preferred.
• Ability to manage high-risk patients and coordinate care plans.
• Strong teamwork, communication, and documentation practices.

Responsabilités

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care activities with the interdisciplinary team.
• Communicate with case managers, coordinators, and pharmacists.
• Participate in team meetings to discuss progress and plans.
• Document care management accurately and ensure regulatory compliance.
• Provide patient education and self-management resources.
• Adhere to HIPAA, Medicare guidelines, and related regulations.
• Travel between offices, facilities, and patient homes as needed.
