Job summary

Your Health Organization is seeking a Nurse Practitioner to serve patients in Cheraw and surrounding areas. You will deliver comprehensive care in patients' homes, coordinate with the care team, and manage high-risk patients to improve outcomes.

This full-time, salary-based position offers opportunities for a monthly bonus and growth within a leading physician group serving SC and GA across homes, facilities, clinics, and telehealth services.

Qualifications

• Licensed NP/PA/Physician license in the employment state, in good standing.
• Experience in patient care, preferably in home health, clinic, facility, telehealth, or specialty care setting.
• Strong communication and interpersonal skills for teamwork.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care with interdisciplinary team to ensure continuity and outcomes.
• Communicate with case managers, clinical coordinators, social workers, pharmacists, health coaches and others to coordinate care.
• Participate in interdisciplinary team meetings to discuss patient progress and plans.
• Document care management activities for accuracy and regulatory compliance.
• Educate patients and families about conditions, plans, and self-management.
• Adhere to HIPAA, Medicare guidelines, and other regulations.
• Travel between offices and care settings to provide patient-centered care.
