Your Health, a physician group serving South Carolina and Georgia, seeks a Nurse Practitioner to deliver home-based care in the Greenwood area. You will join a care team, evaluating high-risk patients, coordinating plans, and managing care to improve outcomes across homes, clinics, and telehealth.

This full-time, salary-based position offers a monthly bonus and requires flexibility for 12-hour or 8-hour shifts, with travel between offices and facilities as needed.

Qualifications

• Licensed NP/PA/Physician with valid license in the state of employment.
• Experience in home health, clinics, facilities, telehealth, or specialty care settings preferred.
• Strong communication and teamwork abilities; ability to work in interdisciplinary teams.
• Ability to evaluate high-risk patients and develop care plans; coordinate activities to improve outcomes.

Responsabilités

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care with interdisciplinary team for continuity of care.
• Communicate with case managers, coordinators, specialists, and pharmacists to coordinate patient care.
• Participate in team meetings to discuss progress and care coordination.
• Document care activities accurately for regulatory compliance.
• Educate patients and families on conditions, treatment plans, and self-management.
