Job summary

Your Health Organization is seeking a Nurse Practitioner to deliver high-quality patient care in homes, clinics, and telehealth settings across South Carolina and Georgia. You will evaluate high-risk patients, coordinate care plans, and collaborate with a multidisciplinary team to improve outcomes.

We offer a full-time salary-based role with monthly bonus opportunities, competitive benefits, and a company vehicle. Travel between offices and care sites is expected as part of the role.

Qualifications

• Evaluate high-risk patients and develop care plans.
• Coordinate care with interdisciplinary team for continuity.
• Communicate with case managers, coordinators, pharmacists, and others.
• Participate in team meetings to discuss patient progress and plans.
• Document care activities to support plans and regulatory compliance.
• Educate patients and families on conditions, treatment, and self-management.
• Adhere to HIPAA, Medicare guidelines, and related regulations.

Responsibilities

• Evaluate high-risk patients with complex medical needs and develop care plans.
• Coordinate care activities with interdisciplinary team to ensure continuity of care.
• Communicate regularly with case managers, clinical coordinators, social workers, specialists, pharmacists, health coaches, etc.
• Participate in interdisciplinary team meetings to discuss patient progress.
• Foster effective communication and collaboration for seamless patient management.
• Meet staff efficiency expectations while delivering high-quality care at homes, clinics, or telehealth.
• Document care management activities accurately to support care plans and compliance.
• Provide patients with education and resources for their conditions and prevention.
