Qualifications

• Nurse Practitioner, PA, or physician with an SC license in good standing.
• Experience in home health, clinics, telehealth, or facility-based settings preferred.
• Strong communication and collaboration within interdisciplinary teams.

Responsibilities

• Evaluate high-risk patients and develop individualized care plans.
• Coordinate care activities with the interdisciplinary team to ensure continuity.
• Communicate with case managers, clinicians, pharmacists, and other team members to coordinate care.
• Participate in team meetings to review patient progress and treatment plans.
• Document care activities accurately to support care plans and compliance.
• Provide patient education and resources for self-management and preventive care.
