Qualifikationen

• Nurse Practitioner, PA, or Physician with valid SC license.
• Experience in home health, clinic, facility, telehealth, or specialty care.
• Strong communication and interpersonal skills for interdisciplinary teams.
• Ability to evaluate high-risk patients and develop care plans.
• Willingness to travel between offices and patient homes.

Aufgaben

• Evaluate high-risk patients with complex medical needs and develop individualized care plans.
• Coordinate care activities with interdisciplinary team for continuity of care.
• Communicate with case managers, clinical coordinators, social workers, specialists, pharmacists, and other team members.
• Participate in interdisciplinary team meetings to discuss patient progress and care coordination.
• Document care management activities accurately to support patient plans and compliance.
• Provide patients with education and resources related to their conditions and preventive care.
• Adhere to HIPAA and Medicare guidelines and monitor quality measures.
