Job summary

Your Health in South Carolina is seeking a Nurse Practitioner to deliver in-home primary care across Cheraw, Chesterfield, Pageland, and nearby communities. You will evaluate high-risk patients, design care plans, and coordinate with an interdisciplinary team to improve outcomes.

This full-time, salary-based position offers a monthly bonus and opportunities for growth, with 12-hour (7 on/7 off) or 8-hour (Mon-Fri) shifts and travel between patient homes and facilities as needed.

Qualifications

• Current NP license in good standing in SC.
• Experience in home health, telehealth, or clinical settings preferred.
• Strong communication and collaboration skills with interdisciplinary teams.

Responsibilities

• Evaluate high-risk patients with complex needs and develop individualized care plans.
• Coordinate care activities with the interdisciplinary team to ensure continuity of care.
• Communicate regularly with case managers, coordinators, and other health professionals to coordinate patient care.
• Participate in team meetings to discuss progress, plans, and care coordination.
• Document care management activities accurately to support patient care plans and compliance.
• Provide patient education and resources related to conditions, treatment plans, and preventive care.
