Job summary

Optum Home & Community Care is hiring a Nurse Practitioner for in‑home visits across Amherst, Appomattox, Campbell, and Lynchburg Counties, VA. This HouseCalls role focuses on preventive care, education, and coordination with primary care providers.

The position offers flexible scheduling (8am–8pm across a 7‑day week), no on‑call, no weekend or holiday requirements, and the opportunity to impact health outcomes through education and gap closure.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric or Adult‑Gerontology Primary Care.
• For PAs: NCCPA certification and state licensure.
• Active, unrestricted licensure in state of practice (or ability to obtain by start date).
• Driver’s license and reliable transportation for home visits.
• Ability to navigate varied home environments during assessments.
• Eligibility to practice independently per state regulations (if applicable).
• 1+ years clinical experience in family/geriatric/home health preferred

Responsibilities

• Review past medical history, medications, vitals and conduct physical exams.
• Perform screenings and point‑of‑care testing as appropriate.
• Identify diagnoses for care management and treatment plans.
• Share findings with PCPs to close care gaps.
• Recognize urgent situations and intervene as needed.
• Educate patients on diseases, medications and adherence.
• Address social determinants of health and provide referrals.
