Job summary

UnitedHealth Group's Optum Home & Community Care is hiring for a HouseCalls Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health assessments for Medicare Advantage members in VA, Accomack and Northampton Counties. This field-based role emphasizes education, gap closure, and collaboration with primary care providers.

The position offers a flexible, part-time schedule (24 HPW) with no on-call, weekends, or holidays, a sign-on bonus or student loan

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• PA applicants must have NCCPA certification and state licensure.
• Unrestricted licensure in the practicing state, with no current disciplinary actions.
• Active driver's license and reliable transportation for home visits.
• Ability to navigate varied home environments during assessments.

Responsibilities

• Conduct comprehensive in-home assessments including medical history, medications, vital signs and exams.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close care gaps.
• Identify urgent or emergent situations and intervene appropriately.
• Educate patients on disease processes, medications and adherence.
• Address social determinants of health and arrange referrals as needed.
