Job summary

Optum Home & Community Care is seeking a part-time Nurse Practitioner to perform in-home health assessments across Accomack and Northampton Counties, VA. The role focuses on preventive care, member education, and care-gap identification in collaboration with PCPs.

The position offers flexibility with 24 hours per week, scheduling between 8 AM and 8 PM, and no on-call, weekends, or holidays. Sign-on or student loan repayment bonuses may be available.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Unrestricted Nurse Practitioner license in Virginia (or ability to obtain before start).
• Active, unrestricted driver’s license and reliable transportation for home visits.
• Ability to perform physical assessment activities in varied home environments.

Responsibilities

• Conduct in-home health assessments for Medicare Advantage and other members.
• Identify care gaps and educate members to improve health outcomes.
• Collaborate with primary care providers to coordinate care and support wellness.
• Provide flexible, part-time field-based services with no on-call duties.
