Job summary

Optum Home & Community Care in Virginia is seeking a field-based Nurse Practitioner to deliver annual in-home health assessments to Medicare Advantage and other plan members. This preventive, non-prescribing role emphasizes education, care gap closure, and collaboration with primary care providers within an interdisciplinary team.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted NP license (or ability to obtain before start date) without disciplinary actions.
• Active, unrestricted driver's license and reliable transportation for home visits.
• Ability to perform assessments in varied home environments (sitting, standing, kneeling).

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plan members.
• Collaborate with PCPs to identify care gaps and improve outcomes.
• Flexible scheduling between 8 AM and 8 PM with 20–40 hours per week.
• No on-call, weekends, or holidays required.
