Job summary

Optum Home & Community Care is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members across VA counties. The role offers flexible scheduling, no on-call, and a 24 HPW part-time arrangement with no weekends or holidays required.

You will document diagnoses, coordinate with PCPs, educate patients, and address social determinants of health, contributing to improved outcomes while enjoying a work-life balance.

Qualifications

• Active ANCC or AANP certification for NP track or NCCPA certification for PA track.
• Unrestricted licensure in practicing state.
• Driver's license and ability to transport to home visits.

Responsibilities

• Conduct in-home assessments for Medicare Advantage and other members.
• Document diagnoses for care management and treatment planning.
• Communicate findings to PCPs to close care gaps.
• Educate members on disease processes, medications, and adherence.
• Address social determinants of health and provide referrals as needed.
• Recognize urgent situations and intervene appropriately.
