Job summary

Optum Home & Community Care, part of UnitedHealth Group, is expanding its HouseCalls program. As a HouseCalls Advanced Practice Clinician (NP/PA), you will conduct annual in-home health assessments for Medicare Advantage and other plan members, delivering education, gap closure, and care coordination with PCPs.

This field-based role offers flexible scheduling (8am–8pm, 24–40 hours/week), no on-call, weekends, or holidays, and a pathway to growth within a supportive interdisciplinary team.

Qualifications

• Active, unrestricted licensure in the practicing state
• Driver's license and reliable transportation for home visits
• Prior home health experience is preferred

Responsibilities

• Review past medical history, medications, vitals and conduct physical exams
• Perform evidence-based screenings and point-of-care testing as appropriate
• Document diagnoses for care management and treatment planning
• Communicate findings to the member's PCP and address gaps in care
• Recognize urgent situations and intervene appropriately
• Educate members on disease processes, meds and adherence
• Address social determinants of health and provide referrals as needed
