Job summary

Optum Home & Community Care (UnitedHealth Group) is seeking a part-time, field-based Nurse Practitioner to perform annual in-home health assessments for Medicare Advantage and other plans across Accomack and Northampton Counties, Virginia. The role emphasizes preventive care, care-gap identification, and collaboration with primary care providers for better outcomes.

The position offers flexible scheduling (8 AM–8 PM), no on-call weekends or holidays, and a $10,000 sign-on or $20,000 student loan

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care.
• Active, unrestricted NP license (or ability to obtain before start).
• Valid driver’s license and reliable transportation for home visits.
• Ability to perform physical assessments in varied home environments.

Responsibilities

• Conduct in-home health assessments and identify care gaps.
• Collaborate with PCPs to improve health outcomes.
• Provide preventive care education and support member wellness.
