Job summary

Humana's CenterWell Home Health is seeking a Patient Services Coordinator-LPN to schedule visits and coordinate care among field staff, patients, physicians, and office teams to maintain continuity of care.

The role supports daily office operations, handles scheduling across multiple patients, and assists with weekly case conferences. A valid state license and at least 1 year of home health experience are preferred.

Qualifications

• Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which he / she practices.
• Must possess excellent communication skills and strong organizational abilities.
• Ability to manage and prioritize multiple assignments in a healthcare setting.
• At least 1 year of home health experience preferred.

Responsibilities

• Manages schedules for all patients and edits schedules when agents are unavailable.
• Initiates infection control forms and handles related documentation.
• Backs up during lunch hours and busy times, handles field staff calls and case conferences.
• Maintains patient hospitalization logs and coordinates notes and emails with staff.
• Schedules add-ons, discharge visits, and OASIS collection, and reschedules as needed.
• Runs scheduling reports and prepares weekly staff schedules.
