Job summary

Humana, Inc. is seeking a Patient Services Coordinator‑LPN in Florida to schedule patient visits and coordinate care with field staff, physicians, and the care team. The role includes assisting with day-to-day office operations and staff management, while ensuring continuity of care.

The position requires an active nursing license (LPN/RN) in the state, at least 1 year of home-health experience, strong communication, and the ability to manage multiple assignments. Scheduled weekly hours are 40.

Qualifications

• Licensed nurse in the applicable state.
• At least 1 year of home-health experience.
• Code certification is preferred.
• Valid state driver’s license and auto liability insurance.
• Excellent communication skills; strong organization and prioritization.

Responsibilities

• Schedule and manage patient visits; reassign patients quickly when staff are unavailable.
• Process infection control forms and route reports to the appropriate leaders.
• Support office operations and staff management, and cover during busy periods or lunch hours.
• Maintain patient hospitalization logs and coordinate with office, field, and sales staff.
