Job summary

CenterWell Home Health is seeking a Patient Services Coordinator LPN to manage patient visit schedules and coordinate with field staff, physicians, and the care team. The role supports day-to-day office operations and staff management to ensure continuity of care.

The ideal candidate is a licensed nurse with at least 1 year of home health experience, strong communication, and organizational skills. Proficiency in scheduling and prioritizing tasks is essential, with on-call rotation as needed.

Qualifications

• Must be a Licensed Professional Nurse or Licensed Vocational Nurse licensed in the state of practice.
• At least 1 year of home health experience.
• Coding certification is preferred.
• Must possess a valid state driver’s license and automobile liability insurance.
• Must be currently licensed in the state of employment.
• Must possess excellent communication skills and strong organizational ability.

Responsibilities

• Schedule visits for patients and coordinate with field staff, physicians and others to maintain care continuity.
• Assist with day-to-day office and staff management.
• Manage and update patient schedules and reassign as needed when staff are unavailable.
• Prepare weekly agent and staff schedules and verify scheduling notes in the system.
• Handle lab reports and route to appropriate medical staff; initiate infection reports when needed.
• Support on-call rotations or clinical visits as required.
• Ensure supervisory visits are scheduled and all required documentation is completed.
• Run scheduling reports and monitor productivity and geographic issues.
