Job summary

Humana Inc, CenterWell Home Health is seeking a Patient Services Coordinator-LPN responsible for scheduling visits and coordinating care among patients, field staff, and physicians. The role also assists with day-to-day office and staff management.

The position requires an active nursing license (LPN/LVN), at least 1 year of home health experience, and strong communication and organizational skills. A valid driver’s license and auto insurance are required.

Qualifications

• Active nursing license in the state of practice (LPN or LVN)
• Minimum 1 year of home health experience
• Excellent communication skills with diverse groups
• Strong organizational and multitasking abilities
• Valid state driver’s license and automobile liability insurance

Responsibilities

• Schedules patient visits and edits schedules for staff shortages.
• Initiates infection control forms and forwards reports to the relevant teams.
• Supports branch staff during peak times and coordinates case conferences.
• Maintains hospitalization and coordination logs and shares updates with staff.
• Ensures supervision visits are scheduled and coordinates add-ons and orders.
• Verifies visit notes in scheduling system and assists with patient transfers.
