Job summary

CenterWell Home Health is seeking a Patient Services Coordinator-LPN to schedule visits and coordinate care with field staff, patients, physicians, etc. to maintain proper care coordination and continuity of care. The role also assists with day-to-day office and staff management.

You will manage patient schedules, ensure timely reassignment when staff are unavailable, and support weekly scheduling activities. A license to practice as an LPN/LVN and 1+ year home health experience are required.

Qualifications

• Be a Licensed Professional Nurse or a Licensed Vocational Nurse licensed in the state in which he / she practices.
• Have at least 1 year of home health experience.
• Prior packet review / QI experience preferred.
• Coding certification is preferred.
• Must possess a valid state driver’s license and automobile liability insurance.
• Must be currently licensed in the State of employment if applicable.
• Must possess excellent communication skills, the ability to interact well with a diverse group of individuals, strong organizational skills, and the ability to manage and prioritize multiple assignments.

Responsibilities

• Manages schedules for all patients and edits schedules for agents calling in sick.
• Initiates infection control forms and submits Employee Infection Reports when needed.
• Backs up during lunch/busy times and assists with weekly case conferences.
• Maintains the client hospitalization log and shares updates with staff.
• Schedules visits, add-ons, and orders; coordinates OASIS collection and recert visits.
• Reschedules missed visits and processes reassigned visits.
• Ensures supervisory visits are scheduled and runs scheduling reports.
